Ground-Level Response and Coordinated Engagement - Whatcom County Health and Community Services

A community-based effort to find solutions for individuals who are high utilizers of emergency and criminal justice systems. The aim is to offer intensive, coordinated services to those whose needs span beyond any single agency. The GRACE Program provides intensive case management, coordination of services, and ongoing support.

Adult Intensive Outpatient Program - Compass Health

A home and community-based, high-intensity treatment service for adults whose recovery and success could benefit from more frequent contact with their clinical team. Services are designed to assist individuals with severe and persistent behavioral health disorders to accomplish their recovery goals and reside successfully in the community.

Law Enforcement Assisted Diversion - Whatcom County Health and Community Services

A community-based effort to find solutions for individuals who are high utilizers of criminal justice systems. The aim is to lead people out of the criminal justice system and into intensive case management where those who are dealing with behavioral health issues can have their needs met appropriately.

Community Case Management - Lake Whatcom Center

Maternity Support Services and Infant Case Management - Answers Counseling

Pregnant individuals and parents of infants who have Apple Health/ Medicaid health insurance are eligible to receive: assistance applying for SNAP/ Basic Food benefits, Childcare Subsidy, and PFML benefits; nutritionist advice; help accessing medical and behavioral health care; and monthly gift cards for participating in Culturally Responsive Integrated & Strength-Based Parenting (CRISP) classes.

An award winning, evidence‐informed home visitation case‐management model for pregnant and parenting moms with substance use disorders. This program serves pregnant and parenting moms who are self-disclosing to engaging in substance use during pregnancy. Pregnant and parenting moms may be admitted to the program from pregnancy or up to 24 months postpartum

Community Engagement Service - Max Higbee Center
