Services are designed to assist individuals with severe and persistent behavioral health disorders to accomplish their recovery goals and reside successfully in the community.

Improving quality of life by coordinating health and social services. Services include comprehensive care management, care coordination and health promotion, transition planning, individual and family support, referral to relevant community and social support services, and health education.

A faith-based addiction recovery program for women. In addition to shelter and relapse prevention, services include one-on-one case management, family focused care plans, connections to resources, life skills classes, work therapy, behavioral health referrals, and more.

If you are needing assistance to live safely in your own home and meet income qualifications, you could be eligible to receive in-home care.

Pairs applicants with community health workers to better help people access care and overcome barriers that prevent them from getting the care they need.

A voluntary program for current or former foster youth, aged 15 through 22, who would like to receive supportive case management to establish and achieve goals in various areas of their lives.

A community-based effort to find solutions for individuals who are high utilizers of emergency and criminal justice systems. The aim is to offer intensive, coordinated services to those whose needs span beyond any single agency. The GRACE Program provides intensive case management, coordination of services, and ongoing support.

People experiencing homelessness can receive services including short-term case management, connection to resources, and some basic needs. Check in for schedule details.

CFI is contracted to work with veterans who need long-term case management and independent living skills training due to a brain injury, spinal cord injury, or other neurological disorder. Provides community-based services and resources to assist veterans as they seek greater independence in their homes and communities.

Pregnant individuals and parents of infants who have Apple Health/ Medicaid health insurance are eligible to receive: assistance applying for SNAP/ Basic Food benefits, Childcare Subsidy, and PFML benefits; nutritionist advice; help accessing medical and behavioral health care; and monthly gift cards for participating in Culturally Responsive Integrated & Strength-Based Parenting (CRISP) classes.

All case management services are free and voluntary and are offered to biological relative and foster caregivers. Services include case management for families navigating a crisis or the after-effects of trauma, and on-call services.
