This program serves pregnant and parenting moms who are self-disclosing to engaging in substance use during pregnancy. Pregnant and parenting moms may be admitted to the program from pregnancy or up to 24 months postpartum

A specialty team that partners closely with many community partner agencies as part of a continuum of services, engaging clients seeking housing support who are either unenrolled from a health plan or seeking enrollment. The CORS Team aims to help clients access housing resources and other necessary services. Services in this program include Care Coordination, Housing Referral and Support, and Outreach and Engagement.

Long-Term Care services in the comfort of your own home. Caregiving services, assistance with activities of daily living, nursing services, assistive technology, and even modifications to your house are aspects of in-home care. If you are needing assistance to live safely in your own home and meet income qualifications, you could be eligible to receive in-home care.

A community-based effort to find solutions for individuals who are high utilizers of emergency and criminal justice systems. The aim is to offer intensive, coordinated services to those whose needs span beyond any single agency. The GRACE Program provides intensive case management, coordination of services, and ongoing support.

People experiencing homelessness can receive services including short-term case management, connection to resources, and some basic needs. Housing case management is provided when Opportunity Council are on-site, check in for schedule details.

Intensive outpatient mental health program. Provides home and community based wraparound services for Medicaid-eligible youth (up to 20 years) with multiple and complex needs.

Social workers are able to help Veterans, families and caregivers with almost any need, from help with finances or housing to questions about treatment to just feeling overwhelmed.

All case management services are free and voluntary and are offered to biological relative and foster caregivers. Services include case management for families navigating a crisis or the after-effects of trauma, and on-call services.

Pregnant individuals and parents of infants who have Apple Health/ Medicaid health insurance are eligible to receive: assistance applying for SNAP/ Basic Food benefits, Childcare Subsidy, and PFML benefits; nutritionist advice; help accessing medical and behavioral health care; and monthly gift cards for participating in Culturally Responsive Integrated & Strength-Based Parenting (CRISP) classes.

Services are designed to assist individuals with severe and persistent behavioral health disorders to accomplish their recovery goals and reside successfully in the community.
