A home and community-based, high-intensity treatment service for adults whose recovery and success could benefit from more frequent contact with their clinical team. Services are designed to assist individuals with severe and persistent behavioral health disorders to accomplish their recovery goals and reside successfully in the community.

Pregnant individuals and parents of infants who have Apple Health/ Medicaid health insurance are eligible to receive: assistance applying for SNAP/ Basic Food benefits, Childcare Subsidy, and PFML benefits; nutritionist advice; help accessing medical and behavioral health care; and monthly gift cards for participating in Culturally Responsive Integrated & Strength-Based Parenting (CRISP) classes.

A community-based effort to find solutions for individuals who are high utilizers of emergency and criminal justice systems. The aim is to offer intensive, coordinated services to those whose needs span beyond any single agency. The GRACE Program provides intensive case management, coordination of services, and ongoing support.

Max Higbee Center members are paired with a Community Engagement provider to receive one-on-one support to introduce community resources and create lasting relationships, develop goals and strategies, and learn skills that will facilitate integration into the community. Long-Term Care services in the comfort of your own home. Caregiving services, assistance with activities of daily living, nursing services, assistive technology, and even modifications to your house are aspects of in-home care. If you are needing assistance to live safely in your own home and meet income qualifications, you could be eligible to receive in-home care.

The aim is to lead people out of the criminal justice system and into intensive case management where those who are dealing with behavioral health issues can have their needs met appropriately.

Financial assistance for eligible workers to pay for tuition, books, and other related items. Includes case management with retraining staff to help understand what paperwork is need, coordinate financial aid, navigate unemployment, and participate in job prep.

Intensive outpatient mental health program. Provides home and community based wraparound services for Medicaid-eligible youth (up to 20 years) with multiple and complex needs.

Eligible families can receive assistance with employment and housing, benefit navigation and application assistance, affordable Internet connectivity, and more.

Supports clients with chronic mental illness to develop skills to manage their symptoms and assists them to access a full range of services in the community. The program's intensity is continually adjusted to address the full spectrum of treatment, from crisis intervention to independence of case management.
