Geriatric Transition Program - Sunrise Health Homes and GTP Services

Helps adults with challenging behaviors transition from a hospital to a long term care facility that meets their needs. This program is for adults with serious mental illness, dementia, mood disorder or other neurological disorders. Eligible adults also have challenging behaviors which are a barrier to hospital discharge OR their long term care facility placement is at risk due to challenging behaviors.

Community Engagement Service - Max Higbee Center

Max Higbee Center members are paired with a Community Engagement provider to receive one-on-one support to introduce community resources and create lasting relationships, develop goals and strategies, and learn skills that will facilitate integration into the community. This program is contracted through DDA and must be approved by your case manager prior to program start.

Transitional Living Program - Northwest Youth Services

A short-term housing program for homeless young adults (single, couples, or parenting) ages 18-24 to work on tenancy skills and housing qualifications. Northwest Youth Services supports youth with: An apartment for up to 18 months, help to find a job, access to health/support services, opening a savings account to save for long term housing.

For youth 14-24 who are working with CFI on individual goal plans. Family-to-Family Class - NAMI of Whatcom County

The free 8-session, 4 week course is taught by trained family members and is for families, partners and friends of individuals with mental illnesses. This program is for young people, ages 18-24, who are moving out of unstable situations that resulted in them being temporarily homeless. This program offers lease support, housing financial assistance, deposit, and ongoing skill building in collaboration with Housing Case Managers.\nNote: Rapid Rehousing is accessed through a main application process and is NOT first come, first served.

Peer-to-Peer Class - NAMI of Whatcom County

This free class is an experiential learning program for people experiencing a mental health challenge who are interested in establishing and maintaining their wellness and recovery. Peer-to-Peer consists of eight two-hour units and is taught by a team of two trained teachers and a volunteer support person, all of whom are personally experienced at living well with mental illness.

English for Speakers of Other Languages Classes - Goodwill Job Training and Education Center

ESOL classes offered at basic and intermediate levels for people who want to develop basic English literacy and improve language skills for the workplace. Classes include ESOL Basic through Level 4, and Citizenship Test Preparation. Classes are held two days a week for 2 hours.

Virtual Intensive Outpatient Program - Charlie Health

Intensive outpatient programming (IOP) is a structured behavioral healthcare that offers a personalized combination of support groups, counseling, family therapy, and individualized therapy. Virtual IOP is meant to address issues such as anxiety, behavioral issues, depression, dissociative disorders, gender identity & dysphoria, mood disorders, personality disorders, self harm, substance use, and other trauma. GED/High School Completion Classes - Goodwill Job Training and Education Center

Goodwill offers two different programs to help people get their high school diploma or GED. High School 21+ program allows colleges to grant high school diplomas to individuals who are 21 years or older. Basic Skills & GED Test Preparation classes helps students prepare for all four GED test subject areas (writing, math, science, and social studies) as well as improving test-taking skills, digital skills, and taking practice tests.

Adult Dental Care - Unity Care NW Ferndale

Prioritizes dental care for the following groups: pregnant women, parents of UCNW pediatric patients, established UCNW pediatric dental patients once they reach adulthood, people living with HIV, developmentally disabled patients, adults with dental emergencies, UCNW medical patients with certain health conditions who are referred by their provider.\nEmergency dental care is available to anyone living in Whatcom County.
