Community First Choice Personal Care - Apple Health
A state Medicaid program (formerly Medicaid Personal Care, or MPC) for people on Medicaid who need help with activities of daily living, including eating, dressing, bathing, or mobility. The program is administered by the Developmental Disabilities Administration (DDA), even for people who are not eligible for DDA. Personal Care services are typically provided in your home.

Application process
For those on Medicaid with a DDA case manager, call your DDA case manager to apply for CFC Personal Care. For those on Medicaid who do not have a DDA case manager, call (360) 714-5000 to apply for CFC Personal Care.
