Provides integration and coordination of health care and other services for those individuals who qualify for this benefit by the Washington State Health Care Authority. Health Home services provided by a Care Coordinator focus on improving the individual’s quality of life, helping to manage multiple providers, assisting with getting appointments, identifying helpful community resources, connecting to available benefits, and helping with post-hospital care.
